A Parent's Guide to Regulated Childcare (must sign)

Medical Report for Childcare (all cooping parents MUST have this signed by medical provider)

Release of Information for Childcare (needs to be filled for cooping parent and notarized)

Emergency Form (we need 2 copies of this to attend school)

Health Inventory (this needs to be taken to pediatrician before attending school)

Picture Waiver Form (gives us permission to display photos of children on website, facebook, printed material, etc)
